OBTAINING COPIES OF DISCIPLINARY ACTIONS

& OTHER RECORDS

You may obtain copies of the disciplinary action(s) taken by the Board against one of the above psychologists or any others disciplined by the Board.  Disciplinary Orders and Minutes of Board meetings at which the case was discussed are available from the Board.  Copies of dismissal letters or letters of concern can also be obtained from the Board.  Note that all materials relating to an investigation by the Board are confidential by law, pursuant to A.R.S. § 32-2082(E) and A.A.C. R4-26-101.  Copies of records are $0.25 per page and you may e-mail the Board at info@psychboard.az.gov or call at (602) 542-8162 to obtain the number of pages and the cost of the documents you wish to receive.  If you wish to obtain records, please complete the form, Request for Public Records and mail it to the Board along with a check or money order for the cost of the records.  Summaries of disciplinary actions can also be found in our newsletter The Examiner.

Audio recordings of Board meetings and hearings are also available for up to six months from the date of the meeting.  The cost of  recordings of Board meetings is $10 per CD, tape or *.wav file (via e-mail).  You may request CDs or tapes by sending the above form and appropriate fee to the Board.
