APPLICATION & RENEWAL FORMS
EXAMINATION and/or LICENSURE: Form for applicants to take the Examination for Professional Practice in Psychology (EPPP), or for psychologists licensed in another state or Canadian province, who do not hold one of the credentials listed below.
LICENSURE BY CREDENTIAL: Form for psychologists who hold a: CPQ, NRHSPP, or ABPP credential, and who are licensed in good standing in another state or Canadian province for at least 5 years.
2009 LICENSE RENEWAL FORMS:
License Renewal form for all Arizona licensees. Please download the form,
read/review the instructions, mail in COMPLETED renewal form, including
the Citizenship Verification Form, along with your associated fee to the
Board office. DO NOT SEND PROOF OF CE AT THIS
TIME!
PLEASE NOTE: There is NO PINK CONFIDENTIAL INFORMATION SHEET, and PROOF OF CITIZENSHIP needs to be accompanied by a COPY of your birth certificate or current passport.
